Appointment Schedule

  • Well child checks occur at 2 weeks, 1 month, 2 months, 4 months, 6 months, 9 months, 12months, 15 months, 18 months, 24 months and 30 months. From age 3 annual physicals are required for your child through their 18th birthday. As always, check with your insurance company for coverage and reimbursement policies.

Immunizations

  • We follow the American Academy of Pediatrics' immunization schedule and ask for you to do your best to comply with the agenda we set for your child. Failure to vaccinate your child in a timely manner will result in dismissal from our practice. For more information about the vaccines your child will be receiving, you may visit the Center for Disease Control and Prevention's website www.cdc.gov or the American Academy of Pediatrics website www.aap.org.

Insurance Card/Patient Information

  • Current demographic sheets must be on file for each patient. All valid insurance information must be provided at time of service. PCP selection must be accurate. Co-pays and all back balances are due prior to appointment. It is your responsibility to notify us of any insurance changes.

Appointment Policies

  • When an appointment is booked, we allot a certain amount of time for your child. If additional problems need to be addressed we will gladly book a subsequent appointment. This is largely due to insurance rules and individual policy regulations and timeliness of visits.

School/ Sports/ Camp Forms

  • Each patient will receive a school/camp form at each annual physical. Please make copies of this record for yourself and for schools and camps. This record is valid for thirteen months from the date of the exam. If you need additional copies after the exam there will be a fee for each request. Please provide a stamped/ self-addressed envelope to mail back to you or pick up in the office within 24 hours of dropping off. We do not fax school/ camp forms.

Scheduling Policies

  • We will do our best to accommodate your scheduling request and will offer an appointment based on availability in both offices. If you are aware that you are unable to make an appointment, please call 24 hours in advance. Failure to notify the office of a cancellation/ same-day cancellations may result in a $25 no-show fee.
  • In consideration of other patients, we may reschedule your visit if you arrive more than 15 minutes late.
  • Co-pays are due at time of visit. Failure to pay will result in an additional fee of $25 to compensate for billing and processing costs.
  • There is a strict limit of 3 no-shows. After the 3rd no-show, a warning/dismissal letter will be mailed to the parent or legal guardian.

Prescriptions

  • We utilize electronic prescribing technology. All prescriptions will be sent electronically, except Class II drugs. Please allow 48 hours for prescription refills. Med checks are required three times per year for Class II/controlled substances.

Insurance Referrals

  • In order to process insurance referrals we need the date of the appointment, physician's full name, phone, FAX and NPI number. Please allow one week for processing.

Before the EmergencyRoom

  • Briarpatch has 24/7 on-call coverage. Please call our office before taking your child to the emergency room, unless you believe he/ she is having a medical emergency. In many circumstances we can provide medical care in our office or at The Evening Pediatric Clinic of Cape Cod. (508)362-KIDS (5437). Saturday and Sunday emergency visits may be scheduled with the on-call doctor's office between 10AM and noon.

Transfer of Records

  • We must have a release signed by the parent/ guardian, or patient if over 18 on file to transfer patient records to a new primary physician. Copies of the record may be picked up or mailed to the new primary care physician. In accordance with Massachusetts law, there is a transfer of recordsfee. Please allow two weeks for processing.
